Service Password

The EZ-Setup, IN-START, etc. menus are all protected by a four digit
password/pin.  On most sets it is 0000, however there are exceptions.

My set asked for a code originally but somewhere along the way as I
have explored it has apparently given up because it no longer asks
for one.

If anyone has any information on other passcodes please
contribute that information for a table to go below here.

And if anyone knows what conditions cause one of these sets to
stop asking, that would probably be interesting as well.

On v3.66 Firmware, the password of the service menu's are changed from
0000 to 0413.
