He's using xinetd to call vsftp, as i understood.

need to verify if listening on port 69.  do, netstat -an;  if we are
answering on this port, try to tftp a file from localhost.  if that's
successful, I guess we're filtering with iptables, or an external firewall
is blockin traffic, (tcp-wrappers would squak in /var/log/messages).  If
no firewall is the problem, check for misconfiguration of tftp file in
/etc/xinetd.d/.  you didn't name the file vsftp did you?  As last resort
try running vsftp in deamon mode.

But i'll bet it's a problem with yer tftp conf file in xinetd.d.
